How we protect your information.

Last updated: [DATE]. This page describes how Allied Integrated Health Services collects, uses, stores, and protects your personal and health information.

Before you publish: this is a starting template — not legal advice. Please have a qualified Ghanaian lawyer review and adapt this page to comply with the Data Protection Act, 2012 (Act 843) and any current Health Insurance regulations.

1. Who we are

Allied Integrated Health Services (“Allied IHS”, “we”, “us”) is a registered healthcare provider in Ghana. We are the data controller for the personal information we hold about you. Our Data Protection Officer can be reached at privacy@alliedihs.com.

2. What information we collect

  • Identity & contact information — name, date of birth, ID number, phone, email, address, emergency contact.
  • Health information — medical history, current conditions, medications, allergies, lifestyle factors relevant to care, visit notes, test results, prescriptions.
  • Insurance & billing information — insurer details, claim history, payments.
  • Information from our website — basic analytics (pages visited, time on site) and any information you enter in forms.

3. How we use your information

  • To provide medical care, including sharing records across our care team.
  • To bill you and your insurer.
  • To contact you about appointments, results, refills, and care reminders.
  • To comply with our legal and regulatory obligations (e.g. reporting notifiable diseases).
  • To improve our services — using anonymised, aggregated information only.

4. Who we share it with

Your information is shared only with people directly involved in your care, or as required by law. Specifically:

  • Other Allied IHS clinicians involved in your care.
  • External laboratories, pharmacies, or specialists you are referred to (with your consent).
  • Your insurer for billing purposes (with your consent at booking).
  • Ghanaian health authorities where required by law.

We never sell your information. We never share it for marketing.

5. How long we keep it

Medical records are retained for the minimum period required by Ghanaian law and good clinical practice (typically [X] years after your last visit), then securely destroyed.

6. Your rights

Under the Data Protection Act 2012, you have the right to access your information, ask us to correct inaccurate information, ask us to delete information where lawful, and lodge a complaint with the Data Protection Commission. To exercise these rights, email privacy@alliedihs.com.

7. Security

We store your information in encrypted systems with restricted access. All staff are trained in patient confidentiality and bound by professional codes of conduct.

8. Changes to this policy

We\’ll update this page if our practices change. The “last updated” date at the top reflects the most recent revision.
