Complete these before you arrive.
Doing your forms ahead of time means your appointment starts when you sit down — not after 20 minutes of clipboards. All forms are available digitally through the patient portal once your appointment is confirmed.
What we ask for
Our standard intake covers:
- Personal & insurance details — name, contact, date of birth, ID number, insurance card details (if applicable), emergency contact.
- Reason for visit — a few sentences in your own words. Don\’t worry about medical terms; describe what you\’re experiencing.
- Medical history — past conditions, surgeries, hospitalisations, family history of significant illness.
- Current medications — including supplements and herbal remedies. If you\’re not sure of names, photograph the boxes.
- Allergies — medications, foods, environmental.
- Lifestyle — a few simple questions on sleep, exercise, alcohol, tobacco. We ask because it changes our recommendations, not to judge.
- Consent & privacy — your acknowledgement of how we use and protect your information.
How long it takes
About 10–15 minutes for a new patient, often less for follow-ups. You can save and return — the form remembers where you left off as long as you\’re signed in to the portal.
If you can’t complete forms online
That’s fine. Arrive 20 minutes before your appointment and our reception team will help you complete a paper version. Bring any medications you take and your ID.
For minors
Parents and guardians complete forms on behalf of children under 16. For young people 16+, they fill in their own forms with a parent’s consent on file.
